“How do I schedule an appointment?”

Call our office at 813-972-2900. (Coming soon: online booking!)

“What do I need to bring with me to my appointment?”

Bring your personal information, Driver’s License or Identification Card as well as your insurance card (if you have one)

“What if I need to reschedule an appointment?”

Please call our office at 813-972-2900 within 24 hours of your scheduled appointment to cancel or reschedule. A $25 no-show fee may apply for failure to contact us within 24 hours.

“Do I need to fast before my appointment?”

If you are a new patient, on any medications, or need labwork/bloodwork drawn, we may require you to be fasting (no food for 12 hours – water is ok to drink) in order to obtain the necessary labwork/bloodwork. Note: some medical conditions may not permit fasting. If you have any questions about fasting, please call ahead of time at 813-972-2900.
